If you would like a copy of your reports and images, please ask one our Associates for a Records Release Form.
We strive to protect our patients' privacy, therefore records will only be release to the patient, legal guardian (if patient is a minor), or Power of Attorney (POA). Please note that legal guardians and Power of Attorney will need to provide appropriate documentation to show that they are in fact the legal guardian or POA. A photo ID will also be required, in order to receive the records.
